Job Title: Remote Social Work Case Manager
Hourly Pay: $25 - $45/hour
Summary:
We are hiring a Social Work Case Manager to provide essential case management services to individuals and families in need. This role involves helping clients navigate social services, healthcare, and personal challenges, developing care plans, advocating for client needs, and ensuring the coordination of services. Key Responsibilities:
- Develop case plans based on clients' social, emotional, and healthcare needs
- Conduct assessments to identify challenges and help clients access resources
- Collaborate with healthcare providers, social services, and community organizations to arrange services
Provide ongoing support and adjust care plans as necessary
- Assist clients with overcoming barriers, such as financial, housing, or healthcare challenges
Advocate for clients in legal, healthcare, or educational settings
Provide crisis intervention and connect clients to urgent resources
- Maintain detailed and confidential records in compliance with regulations
Qualifications:
Bachelor’s degree in Social Work (BSW) required; MSW preferred
3+ years of experience in social work or case management
- Strong understanding of social work principles, ethics, and confidentiality standards
- Excellent communication, problem-solving, and time-management skills
Ability to work independently and collaboratively within a team
- Experience with healthcare systems, community resources, and legal advocacy
- State licensure as a Social Worker (LCSW or equivalent) preferred
