Job Title: Community Transitions Director
Home Office: Vienna, IL or Anna, IL Johnson County Office. Location is dependent on office space availability.
Area of Focus: Behavioral Health
Program(s): Supportive Housing program (820/205) provides adults living with moderate levels of mental illness or disability with 24/7 services with the goal of helping them transition to self-sufficiency or to a less restrictive residential setting. Outpatient Fitness Restoration program (230/260) provides outpatient mental health services and fitness education to non-dangerous defendants who are adjudicated unfit to stand trial. PATH - Projects for Assistance in Transition from Homelessness helps adults experiencing homeless or marginal housing and those living with significant mental illness transition to more stable housing by coordinating services that help them address their individual needs.Housing First (215). The Housing First Program assists adults with severe mental illness who are homeless or exiting a State Operating Psychiatric Hospital in identifying and accessing housing opportunities through the provision of rental and transitional assistance and supports.
Immediate Supervisor: Behavioral Health Officer
Supervises: 820/205, 230/260, PATH and 215 Counselors and Case Managers
Hours of Work: Full-time 40 hours/week. On-call crisis rotation.
Community Transitions Director Minimum Qualifications: Master’s degree in Social Work, Counseling, Psychology, or related human services field. Qualifies as a Qualified mental health Professional (QMHP). Minimum 3 years of experience working in behavioral health with adults with serious mental illness. Minimum 1 year of supervisory or program leadership experience required. Prior experience with transitional programs, supportive housing, or crisis intervention is strongly preferred. Experience working with individuals discharging from psychiatric hospitals or experiencing homelessness is a plus.
Community Transitions Director Skills & Knowledge: In-depth understanding of behavioral health disorders, housing instability, and trauma-informed approaches. Familiarity with Medicaid Rule 140 services (e.g., case management, community support). Excellent knowledge of community resources and systems that support behavioral health recovery and reintegration. Strong supervisory, organizational, communication, and problem-solving skills. Ability to work independently while fostering strong team collaboration.
Community Transitions Director Special Requirements: Must be 21 years of age; maintains a valid driver’s license, reliable transportation and vehicle liability insurance; maintain First Aid & CPR certification; must complete required registry clearances and criminal fingerprint background checks; travel to off-site/community based locations for direct service delivery; schedules evening / or weekend hours as required; availability for on-call responsibilities and emergency response Coordination. Regular travel between Arrowleaf offices and community locations within the southern Illinois service area. Work may include irregular hours or emergency response needs, especially in cases of client crises or urgent discharges. Office-based and field-based work environments.
Community Transitions Director j ob duties include, but are not limited to:
Program & Clinical Oversight
Provide clinical and operational oversight for community-based transition programs in compliance with Medicaid Rule 140 and DMH program guidelines.
Oversee the delivery of case management, counseling, housing navigation, and community support services that facilitate safe and sustainable transitions.
Lead the development and monitoring of individualized service plans and transition plans aligned with person-centered and recovery-oriented principles.
Ensure programs are responsive to individuals discharging from state hospitals, inpatient psychiatric units, shelters, or unstable housing arrangements.
Collaborate with state hospital liaisons, managed care organizations, shelter providers, and other partners to ensure continuity of care.
Staff Supervision & Team Leadership
Supervise and support a multidisciplinary team of case managers, counselors, and peer specialists across multiple programs and service sites.
Provide regular individual and group supervision, coaching staff to navigate complex client needs, risk factors, and system barriers.
Oversee scheduling, performance evaluations, staff development, and onboarding for transition program team members.
Promote clinical integrity, accountability, and staff resilience through a trauma-informed and team-based leadership approach.
Community Collaboration & Resource Navigation
Develop and maintain strong relationships with community-based providers, housing authorities, landlords, shelters, hospitals, and support services.
Serve as a point of contact and problem-solver for barriers to housing placement, benefits access, and wraparound supports for transitioning clients.
Represent Arrowleaf in interagency meetings, hospital discharge planning meetings, and local housing/behavioral health coalitions as needed.
Compliance & Quality Assurance
Monitor program documentation, billing compliance, and outcome tracking to ensure alignment with Medicaid and DMH contract requirements.
Assist in preparing internal reports, grant deliverables, and quality improvement initiatives to evaluate the effectiveness of transition services.
Support incident reporting, utilization review, and continuous quality assurance processes.

S alary Range: $70,120 - $80,475 annually (pay is based on education level, years of experience, and credentials)
