Case Supervisor - Pilsen Area

Healthcare Plus is seeking a dedicated and compassionate Case Supervisor to join our Pilsen team. This is a full-time, Monday through Friday role (8:00 AM - 4:30 PM) where you'll make a real difference in the lives of seniors and their families by ensuring they receive quality care and support.

About the Role

As a Case Supervisor, you will oversee the delivery of care to clients in their homes, guide field staff, and help maintain the high standards of service Healthcare Plus is known for. This position combines office responsibilities with visits to client homes, giving you the opportunity to connect with clients and caregivers while supporting smooth operations behind the scenes.

You will be responsible for:

• Staffing new and existing clients, ensuring schedules are met.

• Supervising and supporting caregivers, including evaluations, coaching, and corrective action when needed.

• Conducting client visits, documenting updates, and ensuring care plans are followed.

• Maintaining accurate records for both clients and employees.

• Partnering with HR, compliance, payroll, and other departments to keep operations running efficiently.

• Participating in community outreach and awareness activities.

What We're Looking For

• A high school diploma or equivalent (required).

• A genuine desire to care for and support the elderly -experience in caregiving or supervision is a plus.

• Strong organizational, communication, and problem-solving skills.

• A reliable vehicle, valid driver's license, and current car insurance (required for traveling between office and client homes).

• The ability to complete Department-sponsored training within 90 days of hire, along with ongoing annual in-service training on aging-related topics.
