- Coordinate care with healthcare teams and facilities to ensure positive outcomes.
- Provide medical oversight, monitor progress, and identify early signs of readmission.
- Arrange specialist evaluations and facilitate seamless patient transitions between care settings.
- Maintain accurate, timely documentation in EMR, including discharge summaries and medication reconciliation.
- Engage patients in health management and develop appropriate treatment plans.
