A Day in the Life of a Hospice Social Worker
As a Hospice Social Worker, your day is centered on connection and care. You’ll meet with patients and families, provide emotional support, coordinate resources, and help navigate the journey of hospice care. As a Social Worker in this role, you’ll collaborate with nurses and the IDG team, advocate for patient needs, and bring calm and clarity during life’s most meaningful moments. Every day as a Hospice Social Worker is different, but always impactful.
What You’ll Do (Hospice Social Worker Responsibilities)
- Assess psychosocial needs of patients and families in hospice care
- Develop care plans and provide counseling as a Social Worker
- Coordinate resources and referrals for patients and caregivers
- Maintain accurate clinical documentation
- Support advance care planning, including DPOA-HC
- Collaborate with the interdisciplinary team to ensure holistic hospice care
- Practice in compliance with Medicare and Illinois Social Work standards
Schedule & Work Environment
- Full-time, onsite position
- Monday through Friday, 8:15am to 4:45pm. Scheduled clinical call rotation includes assigned weeknights and weekends.
- Community-based visits with patients in their homes or care settings
What You Bring (Requirements)
- Master’s degree in Social Work (MSW) from a CSWE-accredited program
- Active Illinois Social Worker license
- At least 1 year of healthcare, palliative care, or hospice experience
- Strong understanding of end-of-life care
- Valid driver’s license, insurance, and reliable transportation
Pay & Benefits
- Salary: $66,314 – $92,839 annually (based on experience)
