About the Role

MedOp Solutions is partnering with a home health organization serving the Central Metro E region to hire an experienced Physical Therapist Case Manager for a contract assignment.

This role is primarily remote, with initial onboarding completed onsite in Oak Brook, Illinois. The Physical Therapist will provide in-home care to adult and geriatric patients, managing a daily caseload while supporting functional recovery and independence.

This is an excellent opportunity for a PT with strong home health experience who is comfortable working independently, traveling within a defined service area, and collaborating with an interdisciplinary care team.

Unit Information

• Setting: Home Health (field-based with remote coordination)
• Census: Approximately 200 patients across the team
• Patient Ratio: 1:1
• Daily Visits: Approximately 6 patients per day
• Coverage Area: Central Metro E (Chicago ZIP codes including 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, 60614 and surrounding areas)

Patient Types

• Adult and geriatric patients
• General medical/surgical conditions
• Patients requiring rehabilitation, mobility support, and functional recovery

Key Responsibilities

• Provide in-home physical therapy services to assigned patients
• Manage a caseload and perform approximately 6 patient visits per day
• Assist patients with activities of daily living (ADLs) and mobility training
• Recommend and train patients on adaptive equipment
• Develop and implement individualized care plans
• Collaborate with interdisciplinary team members (RN, OT, Speech Therapy)
• Document all patient care and visits in EPIC
• Travel within the assigned coverage area to patient homes
• Maintain compliance with home health and regulatory standards

Required Qualifications

• Active Illinois Physical Therapist (PT) license
• BLS certification (AHA) required
• Minimum 2 years of Home Health experience with adult/geriatric patients required
• Strong experience assisting with ADLs and adaptive equipment
• Valid driver’s license required
• Proof of active auto insurance required
• Reliable transportation required
• Ability to work independently in a field-based role

Preferred Qualifications

• Recent home health experience
• Strong interdisciplinary collaboration skills

Support on the Unit

• Nursing supervisor and manager
• Team leads
• Interdisciplinary care team support

Technology & Equipment

• EMR: EPIC
• IV Pumps: Sapphire (if applicable)
• Supplies and equipment delivered to clinician’s home

Orientation

• Approximately 2 days onsite in Oak Brook, IL
• Additional EMR training and skills check-off
• All meetings conducted virtually after onboarding

Scheduling & Expectations

• Monday–Friday, 8:00 AM – 5:00 PM
• No weekends
• No on-call requirements
• No holiday requirements
• Floating within assigned coverage area only
• Must travel to patient homes daily

• Attire: Solid color scrubs or business casual (no jeans or open-toe shoes)
• Transportation: Required for all patient visits
• Parking: Not applicable (field-based role)
