Responsibilities
- Works collaboratively with the Medication Renewal Center nursing and clerical team members to maximize productivity and efficiency.
- Completes medication refill encounters, care due messages, and order entry based on established protocols.
- Provides clear EMR documentation in accordance with department standards regarding medication renewal requests, care due messages, patient care needs, and/or follow-up.
- Receives and enters into the EMR faxed encounters, renewal orders, and other pharmacy communications.
- Orders and schedules appointments and/or lab work as appropriately outlined in protocols.
- Reviews patient compliance with medications through a thorough review of the patient's medical record.
- Communicates medication orders to designated pharmacies via verbal or electronic means.
- Provides general health information using approved references, including individual and group health and wellness education.
- Ensures documentation of care is complete and accurate.
- Maintains and adheres to medication management and refill guidelines and protocols.
- Coordinates with Primary Care Physicians and Nursing staff to meet patient needs.
- Reviews medication renewal requests and care due messages received from pharmacies and patients through various methods, including electronic communications, fax, and phone calls.
- Identifies care gaps and promotes health standards and activities to prevent non-compliance with chronic medication management.
- Reviews current, past, and future appointments and lab work for compliance with safe medication administration.
- Communicates with pharmacies regarding medication discrepancies and/or clarification requests.
- Communicates with patients regarding required services (labs and/or office visits) to facilitate medication refill processes.
- Communicates with patients, providers, care teams, and pharmacies regarding medication changes to reduce pharmacological errors.
- Disseminates patient education as written by the provider.
- Schedules appointments, validates insurance, and processes patient concerns using symptom guides and other department processes when needed.
- Reports medical information obtained from patients accurately, completely, and in a timely manner, ensuring accuracy of information within the EMR.
- Adheres to and enforces Carle policies and procedures.
- Emphasizes attracting, mentoring, and retaining talent through excellent communication, training, and orientation of new personnel following successful completion of the six-month probationary period.
- Provides float support to all Patient Contact Center Clinical teams, including Patient Advisory Nurse (PAN) and Patient Clinical Services (PCS), as volumes, in-baskets, patient care, and other business needs dictate.

Compensation and Benefits The compensation range for this position is $18.33per hour - $29.69per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
