What You’ll Do:
- Coordinate the full admission and transfer process for residents, ensuring smooth transitions and positive experiences
- Lead the Social Services department, supervising and developing team members
- Complete and maintain social service assessments, care plans, and MDS documentation
- Provide support and advocacy to residents and families to enhance well-being and quality of life
- Serve as the abuse and neglect coordinator, ensuring compliance with state and federal regulations
- Collaborate with interdisciplinary teams to drive quality and service excellence
Schedule:
Full-time, salaried positionDay hours with weekend rotation required
What You’ll Need:
- Licensed Clinical Social Worker (LCSW) certification required
- Minimum 3 years of experience in a Skilled Nursing Facility (SNF) with 100+ beds
- Strong leadership, communication, and organizational skills
