We are seeking full-time Physical Therapists to cover the Park Ridge, IL and nearby surrounding suburb and we have multiple openings due to large censes from our Local home health Partners. Coverage is based on where we have active partner home health agencies and current patient referrals, and is not limited to city boundaries.

Most assignments are within a reasonable driving distance from Park Ridge, IL ; however, exact locations may vary based on current caseload distribution.

You will be provided with company-issued technology and all necessary tools to support patient care. Your assigned Manager will assist with supplies, scheduling support, and any ongoing requests .

Compensation Structure

• Evaluation / Re-Evaluation: Base rate
• Routine / Treatment / Follow-Up Visits: $10 less than the evaluation rate
• SOC (OASIS Start of Care): $40 more than the evaluation rate
• ROC/DOC (Recertification / Discharge OASIS): $10 more than the evaluation rate

Caseload & Productivity Expectations

• Full-time productivity expectation: 30 productivity points per week , but still getting paid on per visit basis.

Typical Weekly Visit Mix:

• ~80% Evaluations / Recertifications / Discharges
• ~10% SOC / ROC (OASIS visits)
• ~10% Routine Treatment Visits

Typical Visit Duration (Documentation Included)

• Evaluation: ~1 hour
• Treatment Visit: ~40–45 minutes
• SOC: ~2.5 hours
• ROC / Discharge: ~1+ hour

Pay: $70.00 - $85.00 per hour
