RN Hospice Job Details

• Location: Oak Brook, Illinois and nearby suburbs (Elgin, Bartlett, Carpentersville, Algonquin)
• Employment Type: Locum tenens (Travel/Contract)
• Healthcare Setting: Home health and nursing facilities
• Shift: 8-hour day shifts, Monday–Friday (8:00am–5:00pm)
• Hours per Week: 40
• Contract Duration: 13 weeks, with potential extension
• Caseload: Manage 15–20 hospice patients, averaging 5 patient visits per day
• Scrub Color: Navy
• Parking: Free onsite parking

Registered Nurse Qualifications

• Active Illinois RN license
• BLS certification
• Valid driver’s license and reliable vehicle for travel between patient locations
• Minimum 2 years of recent hospice or end-of-life nursing experience
• Experience with Epic EMR (Remote client and Rover)
• Strong teamwork, adaptability, and flexibility for travel nursing assignments
• Excellent communication and organizational skills

RN Hospice Responsibilities

• Deliver compassionate case management and direct nursing care to hospice patients in home health and nursing facility settings
• Conduct approximately 5 patient visits daily, managing a caseload of 15–20 patients
• Assess, plan, implement, and evaluate individualized care plans for acute symptom management
• Support families and caregivers through end-of-life care, ensuring compliance with hospice regulations
• Document all care provided using Epic EMR (Remote client and Rover)
• Collaborate with triage nurses, team leads, and office staff for care coordination and escalations
• Float to other territories as needed for assessments or coverage
• Participate in a one-week orientation with a preceptor
