Pharmacist - Clinical Anticoagulation Management Services

As an AMS Pharmacist , you’ll command the front lines of anticoagulation management.
Operating under a collaborative practice agreement, you have the autonomy to design, implement, and optimize life-saving therapy regimens for patients across the entire Carle network. You are the ultimate subject matter expert, serving as a critical educator for patients and a trusted consultant for fellow healthcare professionals. From mastering complex bridging protocols to mentoring a dedicated team of pharmacy technicians, you’ll be the driving force behind a gold-standard care program.

Qualifications
Education
- Doctor of Pharmacy (PharmD)
- or Bachelor of Science in Pharmacy with at least 3 years of experience
- Registered Pharmacist (RPh) | Illinois Department of Financial and Professional Regulation (IDFPR)
- Anticoagulation management experience is preferred
- Strong working knowledge of all aspects of clinical guidelines for anticoagulation
Anticoagulation Pharmacist (or Specialist)
Clinical Practice & Patient Care
- Comprehensive Management: Manage warfarin including implementation, monitoring, and peri-procedural bridging protocols.
- Direct Patient Intervention: Provide high-level Medication Therapy Management (MTM) and patient care to optimize prescribing, administration, and clinical monitoring.
- Care Coordination: Facilitate seamless transitions of care, coordinating hospital discharges with inpatient teams and providing pre- and post-procedure guidance.
- Triage & Consultation: Manage high-volume INR reviews and patient encounters; triage abnormal lab results and assess clinical status to determine next steps.
- Patient Education: Counsel patients on therapy adherence, safety considerations, and dosing instructions to improve health outcomes.
- Staff Supervision: Directly supervise Anticoagulation Management Service (AMS) technicians and support staff to ensure operational excellence.
- Guidelines Development: Collaborate with multidisciplinary teams to develop clinical guidelines, order sets, and evidence-based drug therapy protocols.
- Regulatory Compliance: Ensure all pharmacy operations align with the Illinois Pharmacy Practice Act and relevant government/commercial regulations.
- Clinical Safety: Screen for drug-drug, drug-diet, and drug-disease state interactions; provide patient-specific dosing recommendations based on renal function, age, and clinical status.
- Professional Representation: Serve as a pharmacy representative on various hospital committees and workgroups to drive departmental initiatives.
- Staff Development: Lead the training and orientation of new employees and pharmacy staff.
- Communication Management: Oversee patient communication channels, ensuring timely responses to inquiries, triage needs, and education requests.

Compensation and Benefits
The compensation range for this position is $52.06per hour - $89.54per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
