Registered Nurse RN OnCall at Home

: Overview: Sign-On Bonus Eligible! $15,000 with a 2yr work commitment Pay range for this position is $36.25 - $47.79/hour. Actual pay is based on years of licensure. The RN will go out into the field to visit patients in their homes for assessments, meds, admissions, etc. within a 30 min response time of Saint Anthony Medical Center. Fleet car provided. POSITION SUMMARY: The OSF OnCall Hospital at Home registered nurse is a member of a collaborative care team responsible for providing in person care nursing care to a diverse population of adult patients/clients in the patient's home environment. Provides proactive monitoring, prevention, early intervention, and ongoing care with the goal to optimize quality of life for the patient at home through access to professional acute, inpatient healthcare services. Utilizes evidenced-based care and clinical decision support to guide practice. Performs ongoing communication and works to promote quality of care through collaboration with all service team members, patients, and families. Acts as a patient advocate and resource to other health care providers and community-based resources. The RN demonstrates service excellence and accountability through recognizing the direct relationship between cost-effective and efficient provision of care and services including the impact of access to care. Qualifications: REQUIRED QUALIFICATIONS: Education:

• Associates in Nursing

Licenses/Certifications:

• RN-Registered Nurse
• American Heart Association BLS
• Valid current state driver's license

Experience:

• 1 year of recent RN experience in adult acute care population
• 2 years of experience in a role which required effectively communicating and collaborating with health care professionals

Other skills/knowledge:

• Excellent interpersonal and communication skills.
• Solid computer skills, including proficiency with Microsoft software.
• Strong analytical and problem solving skills, with the ability to be detail oriented.

PREFERRED QUALIFICATIONS: Education:

• Bachelor Degree in Nursing

Other requirements/information:

• Proficiency with electronic EMR and communication platforms
• Transportation for travel to/from patients' homes
