Registered Nurse - Bilingual Spanish

The Opportunity

CommunityHealth seeks to hire a full-time Registered Nurse (bilingual Spanish). Under the supervision of the Director of Clinical Services, the Registered Nurse (RN) will provide direct patient care, engaging in education and consultation. The RN seeks to promote, maintain, and restore the health of our patients through intervention and education.

Responsibilities

• Provide direct patient care and patient education regarding medications, lifestyle modifications, and treatment plans through both in person and telehealth based visits.
• Make preliminary assessment of patients’ health problems based on medical history and diagnostic results and prioritize medical attention.
• Review and provide follow up for lab tests and other results.
• Actively participate in quality improvement projects.
• Work with other providers and clinic staff to develop patient plans of care
• Ensure quality standards are met.
• Serve as a preceptor to nursing students and volunteer nurses, as needed.
• Maintain and report data on patient outcomes.
• Triage urgent patient care concerns, as needed.
• Collaborate with medical home team members to coordinate patient care across multiple settings.

Qualifications

• Registered Nurse in the state of Illinois.
• Bachelors degree or higher required.
• One or more years of experience required.
• Bilingual (Spanish/English) required.
• Provide excellent quality of care in overall clinical and patient educational needs.
• Full time position with flexible hours. Some evenings and Saturdays are required.
• Excellent communication and follow-through skills.
• Excellent computer skills (familiarity with Microsoft Word, Excel, and EMR use).
• Must be a self-starter and able to work autonomously.
• Must be flexible and have the ability to manage multiple projects.

Compensation

The starting salary range for this position is $35-$42 per hour ($68,250-$81,900 annually) depending on education and experience.
