Job Details
We are seeking an experienced Hospice Marketing Liaison to support census growth by developing and maintaining referral relationships throughout the Carlsbad and greater North County market. This role focuses on education, relationship management, and representing the organization with professionalism, empathy, and integrity.
Key Responsibilities:
Build and maintain strong referral relationships with hospitals, SNFs, physicians, case managers, and community partners
Educate referral sources on hospice services, eligibility criteria, and care philosophy
Serve as a clinical and operational resource to referral partners
Collaborate closely with admissions, clinical, and leadership teams to support smooth patient transitions
Track referral activity, territory performance, and market trends
Represent the organization at community events, networking opportunities, and healthcare meetings
Ensure all marketing activities align with hospice regulations and compliance standards
Qualifications:
Previous experience in hospice or home health marketing (required)
Strong understanding of hospice eligibility and referral processes
Proven relationship-building and account management skills
Excellent communication, presentation, and follow-up abilities
Self-motivated, organized, and comfortable working independently in the field
Valid driver’s license and reliable transportation
