As a Senior Care Coordinator at Transitions Care you will serve as the lead marketer responsible for both cultivating strong referral relationships with physicians, hospitals, skilled nursing facilities, and other community partners. As a senior member of the business development team, you will also mentor marketers, and collaborate with patients and clinical teams to ensure successful experiences on hospice.
Major Responsibilities
- Develop and manage key accounts and referral sources across assigned territory
- Identify new business opportunities and drive market penetration within priority accounts
- Meet or exceed growth and development targets (minimally acceptable performance of 10 admissions averaged per month) and actively establish and maintain market acceptance and allegiance throughout the local service area.
- Participate in staff meetings, department meetings, team meetings, briefings, in-services, committees, and other related activities as needed.
- Mentor and support Care Coordinators, sharing best practices and coaching in the field.
- Track referral trends and recommend strategy for growth.
- Utilize Trella data. reports and entry information to ensure appropriate documentation is occurring within the team as well as appropriate strategy
- Collaborate closely with clinical and administrative teams to ensure seamless handoffs and timely admissions.
- Represent the organization professionally at community events, in-services, and educational sessions.
Specific Proficiencies
- Meet with team members and learn what their role is with Transitions Hospice.
- Become familiar with the Medicare Hospice Benefit.
- Understand how Transitions Hospice differentiates itself.
- Participate in field rides with key staff members.
- Perfect the Transitions Hospice 101 presentation.
- Begin a territory analysis and build a profile book of contacts.
- Develop and maintain a list of possible objections and how to answer them professionally.

Requirements
Education and Experience
- Minimum 5+ years of hospice or healthcare marketing experience, with hospice strongly preferred.
- Proven track record of meeting and exceeding referral targets in a competitive market
- Excellent communication, relationship building, and strategic thinking skills
- Knowledge of local hospice dynamics, provider landscape, and referral environment
- Ability to work in an interdisciplinary setting
- Experience leading a team preferred but not required
Salary Description
$90,000 - $105,000 a Year
