Field Case Management Coordinator (Illinois)

Location: Illinois (Must reside in Cook, DuPage, Will, Lake, or Winnebago County, IL )
Schedule: Monday – Friday, 8:00 AM – 5:00 PM CST
Job Type: Contract

Pay Rate: Up to $41/hour

Healthcare Consultant III – Field Case Management Coordinator to support members enrolled in Medicare and Medicaid programs. This field-based position focuses on coordinating care for individuals with complex medical and social needs.

This role involves working closely with members, providers, and community organizations to ensure members receive the services and support needed to improve health outcomes. The ideal candidate will have experience in case management, behavioral health, or social services and be comfortable working independently while collaborating virtually with a broader care management team.

Caseloads typically range from 30 to 100 members , depending on complexity and market needs.

Key Responsibilities

• Coordinate and support the case management process for members with complex healthcare needs
• Assist members with appointment scheduling and care coordination
• Identify and help members access healthcare benefits and community resources
• Collaborate with providers, care teams, and community organizations to support member care plans
• Utilize care management tools and internal systems to document member interactions
• Address social determinants of health impacting member wellbeing
• Support discharge planning and transitions of care when applicable
• Maintain accurate documentation and case notes within company systems

Required Qualifications

• Bachelor’s degree required in one of the following fields:
• Behavioral Health
• Social Work
• Psychology
• Counseling
• Marriage & Family Therapy
• Nursing
• Human Services
• OR non-licensed Master’s level clinician
• Minimum 2 years of experience in:
• Behavioral health
• Social services
• Or a related field aligned with program focus
• Must reside in Illinois within one of the following counties:
• Cook County
• DuPage County
• Will County
• Lake County
• Winnebago County
• Ability to work Monday–Friday, 8AM–5PM CST
• Comfortable working independently in a field-based environment

Preferred Qualifications

• Case management experience
• Discharge planning experience
• Managed care experience
• Experience supporting Medicare/Medicaid populations
