Home Health Nurse
Job Title: Registered Nurse (RN) – Home Health
Shift: 8-Hour Days
Job Overview:
Seeking an experienced Home Health RN to provide skilled nursing care to patients in their homes. Responsibilities include patient assessment, care planning, coordination with interdisciplinary teams, and patient/family education.
Key Responsibilities:
- Assess patients (routine & emergent) and develop Plan of Care (POC)
- Deliver and document quality patient care
- Educate patients and families on care, medications, and safety
- Coordinate with care teams and manage case plans
- Supervise support staff (HHA/LVN)
- Ensure compliance with clinical and payer requirements
Requirements:
- Active RN License (state-specific)
- 2+ years Home Health experience (required)
- 1–2 years clinical RN experience
- Homecare Homebase & OASIS experience (required)
- BLS/CPR certification
- Valid driver’s license, reliable transportation & insurance
Preferred:
- CHPN Certification
- BSN
Additional Info:
- Must be comfortable with wound care, IVs, catheter care, and phlebotomy
- Ability to work independently and travel locally
- Strong communication and documentation skills
Education:
- ADN or Nursing Diploma (required)
- BSN (preferred)
