We are seeking a dedicated and compassionate Chronic Registered Nurse (RN) to join our team in La Grange, KY. The ideal candidate will have experience in chronic care management and a commitment to providing high-quality patient-centered care.

Role: Chronic Registered Nurse (RN)
What You'll Do:
Assess and develop individualized care plans for patients with chronic conditions.
Monitor patient progress and adjust treatment plans as necessary.
Collaborate with healthcare teams to ensure comprehensive patient care.
Educate patients and families on disease management and healthy lifestyle choices.
Maintain accurate documentation of patient assessments and interventions.
Participate in quality improvement initiatives to enhance patient outcomes.
What We're Looking For:
✅ Active RN license in the state of Kentucky.
✅ Minimum of 2 years of nursing experience, preferably in chronic care or related fields.
✅ Strong critical thinking skills and attention to detail.
✅ Excellent communication and interpersonal skills.
✅ Ability to work collaboratively within a multidisciplinary team.
