Physical Therapist - Travel Contract

Minimum Requirements

• Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois Physical Therapist license is required to start the assignment. Pending state license accepted.
• BLS certification from AHA or ARC required.
• Minimum 1 year of clinical experience in physical therapy. Home health experience preferred.
• Experience with orthopedic, cardiac, neurologic, pulmonary, and diabetes patient populations preferred.
• Case management skills, ability to work independently, time management and organizational skills required.
• Ability to supervise PTA and function as part of a multidisciplinary team.
• Locals accepted.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: 5x8 schedule, Monday–Friday, approximately 08:00–16:30.
• Typical caseload: 5–6 units per day.
• Patient population: adults and geriatrics only; no pediatric patients.
• Weekend requirement: every other weekend required per client policy.
• On-call: No.
• Floating: No floating to other facilities required.
