Nurse Practitioner/ Physician Assistant

Nurse Practitioner (NP) / Physician Assistant (PA) – Cardiovascular & Vascular Services (Part-Time)
Southwest Suburbs of Chicago

We are a growing cardiovascular practice seeking an experienced Nurse Practitioner or Physician Assistant to provide patient care during nursing home and skilled facility rounds throughout the southwest suburbs of Chicago. This is a flexible, part-time opportunity with competitive compensation and strong growth potential.

The ideal provider will have experience in cardiology, vascular medicine, internal medicine, family medicine, or skilled nursing facility care. Candidates must be comfortable working independently, evaluating patients, coordinating diagnostic testing, and managing follow-up care.

Responsibilities:

• Perform cardiovascular and lower extremity vascular evaluations in nursing home and long-term care facilities
• Screen and evaluate patients for cardiac and vascular conditions
• Develop treatment plans and coordinate patient care with supervising physicians and facility staff
• Order and review diagnostic testing including vascular ultrasounds, echocardiograms, arterial/venous studies, and related cardiovascular exams
• Follow up with patients after procedures, testing, and consultations
• Document patient encounters accurately and efficiently
• Communicate professionally with nursing staff, physicians, patients, and families
• Assist in identifying patients who may benefit from vascular intervention or further cardiovascular evaluation

Qualifications:

• Active Illinois NP or PA license
• Prior experience in cardiology, vascular medicine, internal medicine, family medicine, or SNF/long-term care preferred
• Strong clinical assessment and patient management skills
• Comfortable working independently in multiple facility settings
• Excellent communication and organizational skills
• Reliable transportation required
