Now Hiring: Self-Direction Assistant/QIDP (Case Manager) – Make an Impact!

Pay: $60,000–$62,000 per year
Schedule: Monday–Friday, 8:00 AM to 4:00 PM (Flexibility Required)
Location: Mount Prospect, IL

Overview: This position is responsible for providing Self-Direction Assistance (SDA) to individuals and families, onsite at Search locations, at individuals’ homes, and in the community. The position is based out of the Kensington location in Mount Prospect and travels to locations in northern Cook County.

This position reports to the Enrollments and SDA Manager and requires reliable transportation and some flexibility of schedule.

Responsibilities:

• Provide case management and service facilitation for individuals with SDA funding,
• Schedule required visits in a timely manner,
• Submit billing for services in a timely and accurate manner,
• Manage service agreements for SDA caseload and outside agencies,
• Act as liaison between fiscal agent, employer and personal support workers,
• Conduct outreach and maintain professional contacts,
• Maintain caseload files in a timely, organized manner,
• Demonstrate leadership abilities by working cooperatively with Search colleagues, encouraging teamwork, respecting differing viewpoints and demonstrating the ability to solve problems creatively from an agency-wide perspective,
• Work to maintain budgeted enrollment targets for all Search’s SDA program,
• Present yourself professionally as a representative of Search to prospective participants, family members, guardians, community members, and representatives from affiliated agencies,
• Actively participate and contribute to assigned committees and task forces,
• Manage assigned workload effectively by meeting established deadlines,
• Obtain necessary referral information, and consents, as needed for all referrals,
• Collaborate with DHS representatives and area case coordination units to communicate program vacancies, request referrals, process application packets and secure program or special funding,
• Ensure accurate, timely, and complete participant information is maintained in the electronic caseload database,
• Participate in interdisciplinary teams to develop person-centered plans as needed,
• Perform other duties as assigned by supervisor.

Minimum Requirements:

• Meets DHS requirements to be a QIDP in Illinois,
• Experience as a Self-Direction Assistant, preferred,
• Strong written & verbal communication and presentation skills,
• Basic computer proficiencies including familiarity with Microsoft programs,
• Familiarity with applicable regulations and standards, including DHS Rules 120, 119, 116, 115, and CQL standards, as well as the HCBS Settings Rule,
• Ability to perform basic physical moves such as kneeling, reaching, crouching, bending, and twisting,
• Ability to lift/carry up to 44 pounds,
• Valid driver’s license, proof of insurance and reliable transportation.
