Special Education Case Manager
Educated through skilled learning programs and a charter school for grades Pre-K to 8 that specializes in preparing children to lead in a global world.

We are seeking a Special Education Case Manager Department Job Function: Passages Elementary School serves families and their students from kindergarten through eighth grade. We welcome all students living in Chicago and are one of the most diverse schools representing over 40 languages. Passages offers a tailored educational program focused on high academic standards and instruction based on students" individual needs. Job Function: The case manager ensures that all special education timelines and due process procedures comply with federal and state regulations. The case manager will collaborate and build strong trusting relationships with administration, other school leaders, teachers, community leaders, families, partners, and volunteers. They will serve as a member of the administration team and are responsible for supporting educators so that students with IEPs are supported and meet their IEP goals. Responsibilities include but are not limited to: Ensure all staff understand IEP process
Provide professional development regarding proper IEP documentation for all staff.
Develop special education teachers and parprofessional"s schedules.
Help ensure high level management and administration of IEP binders, scheduling, and SECA justification.
Design and execute professional development around proper IEP document completion for all staff and SPED team specific.
Support special education teachers and paraprofessionals through instructional coaching in order to meet the needs of diverse learners.
Support the implementation of co-teaching models and inclusive practices.
With the Behavioral Health Team, develop and implement the MTSS referral process
Review the MTSS process monthly with administration.
Be an active member of the Behavioral Health Team.
Facilitate regular special education team meetings.
Attend monthly case manager meetings with Chicago Public Schools and collaborate with school leadership to ensure school compliance.
Collaborate with peers to meet multiple demands of school operations and enhance the instructional environment.
Model professional and ethical standards when dealing with students, parents, peers, and community.
Ensure that all procedures and timelines are met 100% of the time by everyone involved in the referral and IEP process.
Ensure that parents are notified of meetings (create notice of conference to send to families).
Facilitate IEP meeting and initiate re-evaluation meetings.
Monitor the evaluation and re-evaluation process.
Monitor the IEP Implementation (ensure minutes are being met).
Notify all staff who have contact with a student regarding their individual responsibilities for that student.
Communicate with general education teachers who will be, or are, working with students.
Provide teacher access to each students IEP with whom they are working; confer with teacher(s) if services/accommodations are not implemented as indicated on the IEP.
Collect, analyze and report data in a meaningful manner every quarter.
Utilize Aspen/SSM at a high level (complete documents in SSM for referrals from teachers or other IEP related documents that arent completed by special education teachers).
Manage all confidential documents, such as IEPs and evaluations, in a secure location.
Keep systematic data on progress monitoring of IEP goals as indicated on the students IEP.
Manage the report card grading policies and implementation of such policies.
Provide parents with progress reports on IEP goals & objectives as indicated in the IEP.
Schedule all annual 504, IEP, and 3-year evaluation meetings (domain, referral, etc).
Manage documentation in permanent files with office staff.
Field questions from staff around IEP processes.
Collaborate with clinical team about annual meetings and referrals.
Assist with crisis management and behavior support for students with IEPs and 504s.
Communicate at least weekly with parents and families Qualifications (Skills/Knowledge/Abilities): Bachelors degree.
LBS1 Endorsement required.
Valid ISBE Special Education endorsement.
Knowledge of state and federal special education laws to ensure the compliance of the school with regulatory requirements.
Ability to clearly communicate with families, students, and staff regarding individual student needs.
Ability to collaborate with classroom teachers to support all students.
Strong coordination and collaboration skills to organize and schedule the special services of all qualified students.
Ability to regularly sit, stand, walk, talk, hear, operate a computer, hand-held learning devices and other office equipment, and reach with hands and arms.
Must occasionally lift and/or move up to 20 pounds.
Must provide proof of Covid19 vaccination and booster, or approved Medical, or Religious, exemption, form provided by our HR department upon request.
