Case Manager

Case Manager – Reset Chicago
The Case Manager of Reset provides weekly case management support for participants in Reset services in collaboration with the Reset team, interns, and volunteers. The Case Manager provides supplemental support to NH programming as needed. The ideal candidate shows a high level of relational and mentoring commitment to women at risk for commercial sexual exploitation and/or currently or formerly in the life. Additionally, the ideal candidate is well-organized and demonstrates a high standard of professionalism. The case manager maintains a case load of up to 12 participants between services and is available to assist participants via individual appointments, care collaboration and as needed support.
The CM is a FT position of 40 hours a week. The CM reports to the Reset Chicago Program Manager.
Office Location: Chicago and Wheaton
Direct Care Services
- Manages case load of Reset participants as assigned by the Program Manager
- Implements the structure of the drop-in services in partnership with interns and volunteers
- Provides clear direction for clients attending drop in services
- Maintains participant engagement through managing the drop-in phone during community hours
- Works with participants to provide transportation to services and appropriate case management appointments as needed
- Collaborates care with the Program Manager, Clinical Care Specialist, and Reset team to support participants in achieving their goals according to their Care Plan
- Participates in participant reviews to track progress and program advancement with the Program Manager and Clinical Care Specialist
- Refers participants to appropriate community services during case management/drop-in hours
- Maintains participant records with precision and accuracy while adhering to HIPAA standards
- Utilizes the knowledge of NH philosophy of care in trauma informed and strengths-based approaches that are participant-led
- Holds spiritual leadership in the drop-in space by offering prayer and encouragement during meal/community times
- Leads community-based support groups with local partners as assigned
- Meets regularly with the Program Manager of Reset for supervision and assessment
- Assess group needs in partnership with the Clinical Care Specialist, utilizing a needs-based assessment survey and participant feedback for program adjustments and growth
- Attends scheduled team meetings and provides participant updates for care team
Community Engagement
- Collaborates with community partners participants in various programs to bring awareness to the community about Naomi’s House services and develop partnerships to support Naomi’s House participants
- Maintains expertise in the field of anti-trafficking work by staying adept in local laws and services for survivors
- Collaborates care with community partners, therapists, and other service providers to fulfill gaps in services for exploited women
Qualifications:
- A personal and growing relationship with Jesus Christ and be an active member or a regular attender of a church.
- Bachelors in Social Work, Psychology, Human Services or related field.
- Seeks to work with a diverse team of professionals in the field
- Strong oral, written communication skills and listening
- Well-developed ability to engage others in building relationships and sustaining momentum.
- Proven capacity to maintain patience, tolerance and tact with diplomacy in high-pressure situations
- High level of integrity and strong ethical values; must maintain the highest standards of confidentiality with all records, including organizational and individual information
Compensation and Benefits:
- Annual salary starting at $45,000-$50,000 depending on experience and qualifications
