Licensed Practical Nurse (LPN)/Registered Nurse
Position Profile:
This position (LPN) is responsible for providing direct nursing care to residents and to supervise the day to-day nursing activities following the current federal, state, and local standards, guidelines, and regulations that govern long term care and assisted living facilities.
Essential Job Functions/Responsibilities:
Observes the resident for changes in their condition and reports these changes to the appropriate physician and other healthcare professionals, including Nurse supervisor
Direct the day to day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long term care facility
Meet with your assigned nursing staff, as well as supportive personnel, in planning the shift's services, programs, and activities
Interpret the department policies and procedures to personnel, residents, visitors, and government agencies as required
Admit, transfer, and discharge residents as required Complete accident/incident reports as necessary
Chart nurses' notes in an informative and descriptive manner that reflects the care provided to residents, as well as the resident's response to care
Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures
Order prescribed medications and recommend supplies and equipment as necessary, and in accordance with established policies
Review medication administrative records for completeness of information, accuracy in the transcription of physician's orders.
Review resident care plans for appropriate resident goals, problems, approaches, and revisions based on nursing needs. Ensure the nurses' notes reflect that the care plan is being followed when administering nursing care or treatment.
Ensure that all nursing service personnel are knowledgeable of the resident's responsibilities and rights
Review complaints and grievances made by the resident and make a written/oral report to the Executive Director indicating what action(s) were taken to resolve the complaint or grievance.
Personal Responsibilities:
Must possess the ability to make independent decisions when circumstances warrant such action. Must be able to relate information concerning a resident's condition.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public
Must be knowledgeable of nursing and medical practices and procedures as well as the laws, regulations and guidelines governing dietary functions in the long term care facility
Must possess leadership ability and willingness to work harmoniously with and supervise other personnel
Skills:
Demonstrates and maintain a positive professional manner, manages time efficiently and effectively, promotes a team approach, participates in appropriate problem solving methods.
With respect to resident rights, ensures all care is provided with respect and dignity for residents, reports all complaints made by residents and or families to supervisor, reports all allegations of abuse, misappropriation of funds/resident property, or any other compliance items immediately
Maintain confidentiality and discretion when dealing with sensitive information about residents, family, employees, and IPH residence
Must enjoy working with the senior population.
Perform job duties in a manner that aligns with the IPH mission of providing a gracious senior living in a Christian surrounding.
Education:
Must possess a current, unencumbered, active license to practice as an RN or LPN in Illinois
Physical and Mental Requirements:
Must be able to move intermittently throughout the work day
Must be able to cope with the mental and emotional stress of the position Must meet the general health requirements set forth by the policies of IPH
Must be able to push, pull, move, and/or lift a minimum of 75 lbs. to a minimum height of 3 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 20 feet.
May be necessary to assist in the evacuation of residents during emergency situations.
