Physical Therapist

About the Job:

We are looking a full-time Physical Therapist to join our growing rehabilitation clinic. This position is based at our outpatient clinic in Deerfield, IL.

Responsibilities:

• Evaluate patient and gathers appropriate information to determine accurate clinical diagnoses and treatment plan.
• Establish appropriate, functional and objective goals based upon clinical examination.
• Devise detailed treatment and plan of care for patients.
• Provide exceptional patient care and service.
• The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in positive and respectful working relationships.
• Cultivate relationships with patients, doctors and team members.
• Provide continuous communication with referring physicians and other referral sources to ensure a consistent level of care.
• Maintain patient charts, clinical logs and checklists necessary for compliance.
• Maintain patient confidentiality with regard to medical records per company policies and procedures and assures compliance with all HIPAA guidelines.
• Communicate well in a professional setting.
• Perform additional duties as required.

Education and/or Experience:

• Bachelor’s, Masters or Doctorate Degree from an accredited Physical Therapy Program.
• Current State of Illinois Physical Therapy license.
• CPR Certified.

Salary: Up to $85,000 per year.
