Notes : FMS Location: Glenview, IL Class/Area: RN-OR Bill Rate $94 before AV fee Call Bill Rate $10 before AV fee, minimum call back 2hrs Desired Start Date: 3/22 Length of Assignment: 12 wks through end of May Hours per Week: 40hrs per week Shift and Time: M- F x 4 (7-5:30P) Weekend Rotation: on call On-call Requirements: 8-10 (including weekend days) must be within 30 mins of hospital Holidays Requirements: N/A Floating Requirements: N/A Experience Required: Epic, 2+yrs plus years, 1 travel contract under belt ,Proficient in circulating ophthalmology-scrubbing highly preferred and DaVinci Robot- general surgery. License Required: Il Lic Certificates Required: BLS, Client Specific Details: Great strong team working at Glenbrook Hospital, Glenview is 28miles to chicago Orientation: any day during week Credentials: Due Thursday before start date, Need CBC,DS and QuantiFERON gold - within in 90 days Submittal: updated resume showing Ophthalmology experience and robotic experience, skills checklist proficient in Ophthalmology, General surgeries and robotic, 1 reference within the last year from a supervisor, verification of IL lic.
