Position Summary

The Grants Manager oversees the full lifecycle of institutional funding at Tapestry 360 Health, managing federal, state, local, foundation, and corporate grants – from prospect research and proposal development to compliance, reporting, and stewardship. The position requires exceptional writing skills, a detail-oriented approach to project management, while navigating both the technical and strategic aspects of grants in a dynamic healthcare environment. As a key member of the Development team, the Grants Manager collaborates with the Chief Development Officer (CDO), executive leadership, finance, clinical, and program teams, and external consultants to secure funding aligned with Tapestry’s strategic priorities. The Grants Manager ensures strict compliance with all regulatory and funder requirements, including HRSA. The position reports to the Chief Development Officer.

Essential Duties and Responsibilities:

Grant Strategy & Development

  • Identify, evaluate, and pursue funding opportunities aligned with Tapestry’s service lines, including primary care, behavioral health, and enabling services.
  • In collaboration with the CDO, lead the development and submission of competitive grant proposals to federal agencies (e.g., HRSA, Title X), state agencies (e.g., IDPH), and private foundations, including narrative development, outcome measures, budgets, and required attachments.
  • Partner with clinical, program, finance, and data teams to align proposals with strategic goals and translate patient outcomes, quality metrics, and community impact into compelling narratives.
  • Develop detailed project budgets in collaboration with finance, clinical, and program teams, ensuring alignment with operational and programmatic needs.
  • Track submission timelines, ensuring timely delivery, and maintain a comprehensive grants calendar.
  • Maintain and continuously improve grant boilerplate materials, including organizational data, UDS (Uniform Data System) metrics, and program descriptions.

Grants Management & Compliance

  • Manage all post-award grant activities, including implementation tracking, deliverables, and reporting timelines.
  • Ensure compliance with federal (2 CFR 200), state, and private funder requirements, including HRSA program expectations.
  • Monitor grant-funded expenditures in collaboration with finance, clinical, and program teams to ensure allowability, allocability, and budget adherence.
  • Maintain complete, accurate, and audit-ready grant files to support site visits, monitor reviews, and external audits.
  • Coordinate responses to funder inquiries, audits, and compliance reviews.

Reporting, Data & Performance Management

  • Partner with clinical, program, quality, and operations teams to collect and validate data for grant reporting (e.g., UDS (Uniform Guidance) measures, patient outcomes, access metrics).
  • Prepare and submit timely, accurate narrative and financial reports in collaboration with the finance, clinical, and program teams, and external consultants.
  • Track performance against grant objectives and support continuous improvement and renewal strategies.
  • Manage grant tracking systems and reporting tools.
  • Collaborate effectively with outside consultants.

Funder Relations & Stewardship

  • Support the cultivation and stewardship of institutional funders in partnership with the CDO and the Chief Executive Officer (CEO).
  • Coordinate funder site visits, including preparation of briefing materials and impact summaries.
  • Ensure timely acknowledgment, reporting, and communication with funders to strengthen long-term funder relationships.

Qualifications:

Education & Experience

  • Bachelor’s degree required; advanced degree (MPA, MPH, or MBA) preferred.
  • Minimum five (5) years of progressive experience in grant writing and management, with at least two (2) years in a healthcare, public health, or nonprofit setting.
  • Proven success in securing federal and state grants exceeding $500,000 annually.
  • Proven success in securing private foundation grants.
  • Familiarity with HRSA, CDC, SAMHSA, or similar federal grant mechanisms preferred.

Skills & Competencies

  • Exceptional writing, editing, and storytelling abilities.
  • Strong financial literacy and experience preparing and monitoring grant budgets.
  • Knowledge of Uniform Guidance (2 CFR 200) and nonprofit compliance standards.
  • Excellent organizational, analytical, and time management skills.
  • Ability to manage multiple deadlines and projects simultaneously.
  • High emotional intelligence and ability to work collaboratively across teams.
  • Proficiency in Microsoft Office and grants management systems (e.g., EGrAMS, UDS, EHB, DonorPerfect, or similar).

Salary and Benefits:

Compensation: The annual salary range for this position is $75,000 to $100,000, based on experience and qualifications.

Tapestry 360 Health is committed to equitable and transparent pay practices. In accordance with the Illinois Pay Transparency Act, we are disclosing the full salary range for this position. This range represents the potential compensation for the role based on experience, tenure, and performance over time.

Most new employees can expect an initial offer within the lower portion of the range, reflecting factors such as prior experience, internal equity, and organizational budget. Salary progression is evaluated regularly to support professional growth and retention.
