PT Medication Tech/Aide, Hinsdale

Looking for reliable, responsible and compassionate caregivers to assist in giving medications to 32 adult home residents with medical/mental health needs; also to provide basic aide related tasks. Training is fully provided. Full or Part Time. Various shifts (days, afternoons, overnights) and weekdays and/or weekend shifts available. We can work around your schedule. Scope Of Responsibilities: Overall Care of All Residents and Facility from Start of Shift Until Relieved from Shift. Preform Personal Services, Medication Assistance and Recording, Documentation, And Shifts Reports. Be responsible for facility when administrative staff is not present in facility, follow proper procedure on notifying management. Scheduled Daily Tasks: Verbal Report Upon Shift Changes Regarding Residents'/Facility Needs. Count And Record Controlled Medications Always During Shift Change. Review Communication Log to Ensure That You Have Received All Updates Regarding Residents. Personally Check Residents Conditions When Any Change Is in Report. Document Any Changes as Per Policy. Assist With Medication Administration as Per Medication Administration Policy and Procedure. In An Emergency, May Have to Assist PCA/HHA Aid with Personal Care as Needed. Complete And Follow Thru All Medical Update Orders and Arrange Transport When Necessary for Next Appointment. Resident medication audit, when done send to pharmacy to be filled. If Medications Are Delivered from Pharmacy During Your Shift, Follow Medication Delivery Procedure. Record supplies that need to be purchased Additional Task Maybe Requested by Administrative Management as Well 10pm-6am Shift Follows Daily Task Schedule (Routine Specifically for This Shift) Follow End of Shift Procedure/Employee Responsibilities End Of Shift: Sign Resident Mars/Alp Book (Daily Task Sheets) Make Sure You Documented/Reported Changes/Findings in Communication Chart Fill Out Appropriate Forms/Reports When Necessary All Resident Care/Medical Needs Complete Oral Report for Shift Change Count And Record Controlled Medications Always During Shift Change.
