Mobile Advanced Practice Nurse
We are seeking Nurse Practitioners/Physician Assistants with Full Practice Authority to perform in-home/telehealth health assessments for patients in the Chicago area. This is a unique opportunity to work with patients who may be less engaged with traditional healthcare, helping them better understand and access the care they need.
What You’ll Do:
- Conduct comprehensive health assessments in patient homes or via telehealth
- Review and update medical histories and current medications
- Document findings and care plans in EHR
- Identify health risks, chronic conditions, and gaps in care
- Travel within Chicago as needed (mileage reimbursed)
