A-Line Staffing is now hiring a Field Care Management Specialist (LCPC, LCSW or RN) . This will be full time.

• Schedule - Monday – Friday- 8:00 AM – 5:00 PM CST
• Pay Rate - $35.87 an hour + Mileage reimbursement provided
• Benefits – active after 90 days of employment
• Job Type - Project-based assignment with potential extension beyond 90 days based on business need and performance
• Location – Candidates must live in or near TARGET Counties: Cook, Lake, Will, Kane, Winnebago, Rock Island

Responsibilities

• Conduct comprehensive member assessments using clinical judgment, data review, and case management tools to evaluate healthcare needs, eligibility, and required services.
• Develop, implement, and monitor individualized care plans for elderly individuals and members with disabilities receiving in-home or nursing facility care.
• Coordinate services with healthcare providers, community organizations, caregivers, and internal teams to ensure quality, cost-effective outcomes.
• Perform field-based visits to members’ homes and care facilities throughout assigned counties while maintaining accurate and timely documentation.
• Advocate for members by identifying appropriate resources, support services, and benefit options to improve health outcomes and quality of care.
• Apply standardized case management guidelines, regulatory requirements, and company policies to support appropriate administration of benefits and services.
• Collaborate virtually with interdisciplinary teams using corporate systems and communication tools while working independently from a home-based environment.
• Maintain productivity standards and effectively manage caseloads ranging from 30–100 members based on market needs and case complexity.
• Utilize critical thinking and problem-solving skills to address barriers to care, discharge planning needs, and coordination of services.
• Ensure compliance with accreditation standards, state regulations, HIPAA guidelines, and internal quality management processes.
• Accurately document assessments, interventions, care coordination activities, and member progress within multiple computer systems and proprietary applications.

Requirements

• Requires an LCSW or LCPC or RN with unrestricted IL active license.
• Experience with case management.
