Responsibilities

Leadership & Team Supervision

  • Direct Supervision: Provide direct supervision, guidance, and performance management for the Medical Coordinator, ensuring all medical administrative tasks are completed accurately and timely.
  • Workflow Oversight: Oversee the Medical Coordinator’s management of medical appointments, lab tracking, and family communications to ensure clinical follow-through.
  • Staff Development: Supervise and train Individual Support Providers on medication dispensation, health monitoring, and wellness promotion.
  • Team Launching: Lead the clinical setup for new support teams when admitting or transitioning an individual into the program.
  • Clinical Oversight & Case Management
  • Medical & Psychiatric Monitoring: Maintain comprehensive, up-to-date knowledge of the medical and psychiatric needs of all individuals.
  • Service Coordination: Collaborate with the Medical & Benefit Coordinator to ensure residents have access to appropriate medical support, specialty services, and timely follow-up care.
  • Medication Management: Oversee medical orders and medication supply; provide on-call clinical support during medication administration windows.
  • Assessment: Complete initial and ongoing health assessments in strict accordance with DHS regulations.

Training & Compliance (Rule 116)

  • Compliance Leadership: Act as the subject matter expert for Rule 116; lead internal audits and generate corrective action plans to ensure agency compliance.
  • Clinical Education: Facilitate ongoing training regarding medical orders, potential side effects, injury prevention, and general wellness.
  • Quality Improvement: Analyze and present Incident Report data during quarterly Continuous Quality Improvement (CQI) meetings.
  • Collaboration & Advocacy
  • Intake & Transition: Participate in the Residential Supports intake process and present medical recommendations to the Residential Supports Admission Panel.
  • Interdisciplinary Communication: Communicate effectively with physicians, pharmacists, families, and internal program staff.
  • Documentation: Maintain personnel and client files according to agency procedures and DHS regulatory standards.

Decision Making Authority

The Nurse Trainer exercises high-level discretion and independent clinical judgment. They are empowered to determine workflow priorities within the scope of their license and are expected to anticipate and resolve complex health-related issues autonomously.

Requirements

Licensure and certification:

  • Registered Nurse in the State of Illinois.
  • DHS Registered Nurse Trainer (or eligibility to obtain)
  • CPR/First Aid

Experience:

  • Minimum of two (2) years as RN
  • Experience with individuals with Intellectual/Developmental Disabilities (I/DD)

Logistics:

  • Valid Driver’s License, safe driving record, and access to a reliable vehicle.
