Position Overview
The Transition Services Coordinator (TSC) is responsible for supporting participants as they move from program enrollment to employment or registered apprenticeship placement. This role ensures completion of Orientation and Pre-Transition Career Assessments, development of individualized career plans, and delivery of comprehensive career readiness services. The TSC works closely with program staff, employers, Department of Labor (DOL)-registered apprenticeship programs, and community partners to facilitate timely and successful participant transitions. The role also includes post-program follow-up and employer engagement through the Career Pathway Management Platform (CPMP).
Key Responsibilities
Orientation & Career Assessment
- Ensure all participants complete Orientation and required pre-transition career assessments
- Review assessment results and identify career interests, skills, and readiness levels
- Collaborate with program staff to align assessments with participant goals and program pathways
Individualized Career Planning
- Develop individualized career plans in coordination with participants and program staff
- Assist participants with resume development, updates, and customization
- Support participants in identifying employment or apprenticeship pathways aligned with career goals
Career Readiness Services
- Deliver career readiness activities including mock interviews, interview preparation, and job search strategies
- Provide guidance on workplace expectations, professional communication, and job retention skills
- Assist participants with completing DOL-registered apprenticeship applications and related documentation
Employer & Apprenticeship Engagement
- Build and maintain relationships with employers, DOL-registered apprenticeship programs, and workforce partners
- Coordinate interviews, referrals, and placements for program participants
- Respond to and track job postings within the Career Pathway Management Platform (CPMP)
- Support alignment between employer needs and participant qualifications
Transition & Placement Support
- Facilitate timely transition of program graduates into employment, apprenticeships, or advanced training
- Track placements, outcomes, and transition milestones in accordance with grant requirements
- Communicate regularly with employers and partners regarding participant progress and placement status
Post-Program Follow-Up & Documentation
- Conduct required post-program proactive follow-up with graduates to track employment retention and outcomes
- Document follow-up activities, placements, and outcomes accurately and on time
- Maintain organized, audit-ready participant and employer records
Collaboration & Program Support
- Work collaboratively with instructors, Student Support Services Coordinator, Program Administrator, and leadership
- Participate in case conferencing, staff meetings, and performance reviews
- Support overall program compliance, performance metrics, and continuous improvement efforts

Required Qualifications
- Associate’s or Bachelor’s degree in Workforce Development, Human Services, Business, or a related field
- Minimum of 2 years of experience in career coaching, workforce development, job placement, or employer engagement
- Experience supporting job seekers with resumes, interviews, and career planning
- Strong organizational, documentation, and follow-up skill
Preferred Qualifications
- Experience working with DOL-registered apprenticeship programs
- Familiarity with CPMP or similar workforce data management platforms
- Experience working with justice-involved, reentry, or non-traditional populations
- Established relationships with employers or workforce partners
Working Conditions
- May require occasional evening or weekend work to accommodate employer or participant schedules
- Must maintain confidentiality and handle sensitive participant information
