Speech Language Pathologist - Travel Contract

Join a growing home health team in Oak Brook, IL as a Speech-Language Pathologist providing patient-centered care in home settings. Minimum Requirements

• Master‘s Degree in Speech-Language Pathology from an accredited program.
• Certificate of Clinical Competence (CCC) in Speech-Language Pathology from ASHA, or CCC-eligible with intent to obtain CCC. (Required)
• Active Illinois state license to practice speech-language pathology. (Required)
• Proof of current CPR certification. (Required)
• Current driver‘s license, reliable transportation, and personal auto insurance. (Required)
• Minimum 1 year of clinical experience in Speech-Language Pathology. (Required)
• Experience in a home health or home care environment. (Preferred)
• Demonstrated ability to perform patient evaluations, develop and document Plans of Care, and provide caregiver education.
• Ability to perform physical tasks required for home visits, including lifting, bending, and prolonged standing/walking.
• HCHB, OASIS, and home health electronic documentation experience. (Required)
• Ability to work a flexible schedule and travel locally. Float to other local facilities may be required.

Assignment Details

• Facility Type: Home Health.
• Position Type: Per Diem / Temporary Contractor, travel assignment.
• Start/End: Assignment window spans May through August (dates provided by client).
• On-call as needed per branch request. On-call needs are subject to change based on staff and branch needs.
• Expected weekly productivity: 30 units (approx. 6 units per week billed as noted by client).
• Required systems experience: HCHB and OASIS documentation.
