Job Title: Licensed Practical Nurse (LPN) – Long-Term Care
Location: Gifford, IL
Type: Contract
Pay Rate: $30/hr
Shift: Day Shift (6:00 AM – 6:00 PM) | 36 hours/week
Schedule: 3 × 12-hour shifts per week, every other weekend required
Position Overview:
Navitas Healthcare, LLC is seeking Licensed Practical Nurse (LPN) – Long-Term Care for an exciting Travel or Local job in Gifford, IL.
Responsibilities:
- Provide direct nursing care to LTC residents
- Administer medications and treatments within LPN scope of practice
- Monitor patient conditions and report changes to supervising RN
- Assist with daily resident care and support CNA staff
- Document care accurately using PointClickCare (PCC) charting system
- Maintain compliance with facility policies and safety standards
- Collaborate with interdisciplinary care team
Requirements:
- Active LPN license required
- Experience in Long-Term Care (LTC) or nursing home setting preferred
- Ability to work 12-hour shifts and every other weekend
- Strong clinical and communication skills
- Must complete facility orientation and acknowledgment prior to start
