Hospice LPN - Quality Assurance(2 yr. Exp. Req.)(LPN Exp. Req.)

Essential Job Functions/Responsibilities

1. The QA Nurse will audit all charts submitted by our clinicians to assure all are in compliance with State and Federal regulations. This position is primarily office based. In this role you will review all clinical documentation and checking for completion and accuracy. You will be working with the Clinical Director, clinical and office team to support workflow functions that impact chart review.

2. Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.

3. Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.

4. Review all assessment documents for accuracy, consistency and supporting documentation.
5. Review clinical charting for conformance/compliance to Medicare and Agency standards.

6. Discuss any noted documentation discrepancies with Supervisors and clinical staff as appropriate.

7. Participate in quality assurance and improvement activities for the company.

8. Ensures maximum third-party reimbursement through direct oversight of insurance verification and authorization processes.

9. Participates in quality assessment performance improvement teams and activities.

10. Survey Compliance and maintaining readiness .

11. Other duties as assigned.

Position Qualifications

1. Current LPN license in Illinois in good standing.

2. At least one (1) year experience in health care, preferably in hospice operations.

3. Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices.

4. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
