Overview
We are seeking a dedicated professional to oversee the coordination of care for our clients. This position is vital in ensuring that all aspects of client care are managed effectively and compassionately.
Key Responsibilities- Manage and coordinate the delivery of care services to clients.
- Collaborate with healthcare professionals and support staff to ensure comprehensive care plans.
- Monitor client progress and adjust care strategies as necessary.
- Facilitate communication between clients, families, and healthcare providers.
- Ensure compliance with all regulatory requirements and company policies.
- A minimum of a degree in healthcare management, nursing, or a related field.
- Proven experience in care coordination or healthcare management.
- Strong organisational and communication skills.
- Ability to work collaboratively in a team environment.
- Proficient in using electronic health records and other healthcare technology.
- Certification in care coordination or case management.
- Experience working with diverse populations.
- Knowledge of local healthcare resources and services.
