Skilled Nursing Center of Eastern Illinois is seeking a compassionate, experienced, and licensed Director of Social Services to lead the facility's social services department. This leadership role is responsible for overseeing the delivery of social services to residents throughout their stay, ensuring individualized support, regulatory compliance, and collaboration with the interdisciplinary care team.
Key ResponsibilitiesLead and oversee the daily operations of the Social Services department.
Supervise, mentor, and support social services staff, including hiring, onboarding, scheduling, performance management, and professional development.
Conduct psychosocial assessments and coordinate appropriate services based on resident needs.
Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team.
Ensure compliance with all applicable federal, state, and local regulations related to social services.
Maintain accurate and timely documentation in accordance with regulatory standards.
Develop, review, and update departmental policies and procedures.
Identify opportunities to improve workflows, operational efficiency, and resident outcomes.
Provide guidance and support to residents and families regarding care planning, discharge planning, and community resources.
Stay informed of changes in healthcare regulations and professional best practices.
Participate in quality improvement initiatives and facility committees as assigned.
Perform additional duties as needed to support resident care and departmental operations.
Demonstrated leadership and team management experience.
Ability to make sound decisions while exercising independent judgment.
Strong organizational, interpersonal, and communication skills.
Knowledge of social service practices within a skilled nursing or post-acute care setting.
Ability to develop and implement departmental processes and procedures.
Proficiency with computer systems and electronic documentation.
Ability to read, write, and communicate effectively in English.
Bachelor's degree in Social Work or a related human services field required (such as Sociology, Psychology, Gerontology, Rehabilitation Counseling, or Special Education).
Current Social Work license, if required by state regulations.
Master's degree in Social Work (MSW) or a related field preferred.
Previous experience in a skilled nursing, rehabilitation, or long-term care setting is preferred.
Prior supervisory or management experience is highly desirable.
This position requires the ability to walk, stand, bend, reach, lift, push, pull, sit for extended periods, and perform fine motor tasks, with or without reasonable accommodation.
