Job Summary
We are looking for individuals to join our client’s team as Licensed Medical Social Worker. This outpatient program provides comprehensive screening and treatment services for adult survivors of the September 11, 2001, terrorist attacks who are experiencing medical and/or mental health conditions related to their exposure.
The selected candidate will become part of a multidisciplinary mental health team, working alongside social workers, psychologists, psychiatrists, pulmonologists, gastroenterologists, and other healthcare professionals. In this role, you will provide psychosocial assessments, care coordination, and evidence-based support to help patients receive appropriate screening, treatment, and ongoing care for 9/11-related mental health conditions.
Job Description
- Conducting mental health screening assessment interviews utilizing symptom measures, documenting the assessments in Epic and using database, and presenting at weekly team meetings
- Providing psychosocial assessments
- Providing benefits counseling.
- Development of appropriate disposition planning for each patient. Identifying and providing patients with appropriate mental health/social service referrals as needed.
- Participating as part of a multidisciplinary team including both mental health and medical providers toward coordination of care.
- Working closely with related social and community agencies.
- Providing professional and appropriate interventions in crisis or emergency situations.
- Completing appropriate documentation in the electronic medical record.
- Workload: Approximately 25–30 patient assessments per week, including initial (new) assessments, follow-up (monitoring) assessments, mental health/social service referrals, and external provider referrals (up to 10 referrals per week).
Skills Required
- One (1) year of LMSW experience
- Experience working in a mental health clinic or hospital setting.
- Strong preference for experience working with ptsd/trauma-informed
- Dedicated to working with a population coping with persistent mental and medical health conditions;
- Strong clinical skills
- In-depth psychosocial assessment and MH screening experience to include risk assessment
- Organized
- Experience with disposition/case presentation
- Highly conscientious
Education/Training/Certifications
- Minimum Degree Required: Master's Degree
- Valid Licensed Master of Social Work (LMSW) license issued by NYSED required
Additional Requirements
- Shift Time: 9:00 AM-5:00 PM
Pay Rate
- $52- $61 per hour, based on experience, skills, and market factors
