Responsibilities
Administer medications accurately per orders and document administration.
Provide skilled LPN nursing care on med-surg units, including patient assessments and vitals.
Monitor patient status, report changes, and escalate concerns promptly.
Maintain accurate, timely charting and ensure regulatory compliance.
Uphold safety standards, infection control, and patient privacy protocols.
Collaborate with multidisciplinary team to plan and adjust patient care plans.
Utilize performance metrics and pipelines to track patient outcomes and efficiency.
Demonstrate a growth mindset by pursuing ongoing professional development.
Support a high-visibility work environment by communicating progress and results.
Assist with discharge planning and patient education for post-hospital care.
Mentor junior staff and contribute to a collaborative team culture.
Participate in performance reviews and contribute to process improvement initiatives.
Experience/Qualifications
Licensed Practical Nurse (LPN) with current state licensure.
Accurate med administration per orders with proper documentation.
Med-surg experience: 1+ year in inpatient units preferred.
Vital signs, patient assessments, and timely status reporting.
Regulatory compliance and thorough charting in electronic health records.
Infection control, safety, and patient privacy adherence.
Collaborative care planning with multidisciplinary team.
Commitment to professional development and growth mindset.
Performance metrics awareness and ability to use pipelines for outcomes.
Discharge planning and patient education for post-hospital care.
