The job description specifies the following requirements for the Speech Therapist position:- **Licensure and Certifications**:- License/Certificate Verification is required for onboarding.- Professional and licenses/certifications verification is mandatory for onboarding.- **Professional Credentials and Checks**:- A valid Driver's License/State ID for onboarding purposes.- Prime OIG, SAM/EPLS, and Criminal Background Check are necessary for onboarding.- Professional Resume and References (#1 and #2) needed for submission.- Prime Agency Attestation and Employment eligibility verification (Form I-9) are required for onboarding.- **Health and Personal History Requirements**:- COVID-19 Vaccine, MMR, Hepatitis B, Influenza Vaccine, TB Test, Varicella, Tdap, and Physical/Health Statement for onboarding.- Prime Drug Screening (minimum 10-panel test) for onboarding.- Annual TB Questionnaire and Fit Test for onboarding.- **Testing and Competencies**:- Prime Core Competencies Exam and Skills Checklist required for onboarding.- **Documents for Onboarding**:- Social Security Card, Badge Photo, Job Description, Essentials Onboarding Packet, and Onboarding Education Attestation for onboarding.This position also involves performing both inpatient and outpatient tasks, with a preference for a combination schedule of 3 days inpatient and 2 days outpatient, or 5 days inpatient.
