Job Description

We are seeking an experienced Home Health Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes. The RN will manage a daily caseload, perform comprehensive assessments, and deliver patient-centered care in compliance with home health regulations.

Job Duties

• • Provide skilled nursing care to patients in the home setting, including assessment, treatment, and patient education.

  • Perform comprehensive patient assessments and monitor clinical status, reporting changes as needed.

  • Administer medications and IV therapy and manage wound and cardiac-related care.

  • Complete accurate, timely documentation in compliance with home health and regulatory standards.

  • Coordinate care with physicians and interdisciplinary teams to ensure safe, effective patient outcomes.

• Minimum 1 year of acute care RN experience within the past 3 years

• Home health, IV therapy, ER, or oncology experience preferred

• Active Illinois RN license

• BLS certification required
