Responsibilities
- Evaluate needs of assigned patients, establish goals, and implement interventions to promote independence with activities of daily living
- Interact professionally with patients, families, and the health care team; collaborate with physicians on care plans
- Develop and update a plan of care (POC), document during care in the electronic medical record, and ensure timely completion
- Provide supervision to OT/OTA staff and students; uphold professional standards and evidence-based practice per AOTA standards
- Maintain a clean, safe care environment and uphold patient privacy and confidentiality
- Assist with patient transfers/positioning and participate in ongoing professional development and in-service activities
- May participate in Saturday/Sunday rotation as applicable and contribute to ongoing quality improvement

Requirements
- Active Illinois Occupational Therapist license
- BLS (AHA) certification (active at time of submission)
- COVID-19 vaccination required; declinations will not be approved
- Minimum 1 year Home Health OT experience (required)
- OASIS and start-of-care experience strongly preferred
- Valid driver’s license and car insurance (required)
- Education: Degree in Occupational Therapy from an accredited program (as applicable per job notes)
