Flexible Part time schedule!
We are looking for a caring Nurse Practitioner to join our medical practice in the Rockford, IL area providing wound care. We need a compassionate practitioner, who enjoys helping others, and is interested in the field of wound care in a home health and Long-term care settings. The NP will round 1 day a week in a Long-term care facility and will spend at least 2 days a week providing home health wound care services. The incoming NP will complete training with our staff prior to working independently. The days/hours are flexible; however, the NP must be available either Mondays/Wednesdays to round at the long-term care facility.
Job Description:
-Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources
-Evaluates and manages patients for wound care and ensuring delivered care meets current standards of practice
-Adheres to clinical practice guidelines for wound care
-Performs care that is accurate, efficient and consistent with the patient's needs
-Coordinates the patient plan of care with the facility's clinical disciplines to address patient needs, evaluate the effectiveness of care, and assess changes to the patient medical condition to ensure healing outcome goals
-Ensures that education is provided to the agency staff regarding the management of the patient's wound(s)
-Maintains communication and a working relationship with home health agency Administrator, team of health care providers and patient family members regarding wound care services
-Demonstrates competency in the position of wound care including assessment, debridement, wound dressings, skin substitutes and other treatment as needed
-Explains treatment plan to patient and family members
Required Skills:
-Must be certified in Sharps Debridement
-Must be wound care certified.
-Previous wound care experience preferred, however experience similar field will be considered.
-Must have valid NP license in the state of IL
Salary:
-$65 - $70 per hour plus paid milage
