Position SummaryThe Physical Therapist plans, prepares, and implements individualized physical therapy programs to maintain, improve, or restore physical function, alleviate pain, and prevent physical dysfunction in patients of all ages. This role requires clinical expertise, strong communication skills, and a commitment to patient safety, regulatory compliance, and service excellence.
Essential Job FunctionsDemonstrate Vista Medical Center's Standards of Behavior and utilize the AIDET communication model in all interactions.
Comply with all organizational, departmental, and regulatory policies, procedures, and standards.
Participate in staff meetings, in-services, performance improvement activities, and ongoing professional development.
Assess patient status by gathering and interpreting relevant clinical data and completing physical therapy evaluations within 24 hours of referral.
Develop, implement, and modify individualized treatment plans, including therapy duration, frequency, and caregiver education.
Communicate evaluation findings, treatment plans, progress, and prognosis clearly to patients, families, and interdisciplinary team members.
Monitor patient response to treatment and adjust plans of care as clinically indicated.
Safely and competently operate therapy equipment and perform technical procedures in accordance with APTA standards, the Illinois Practice Act, and Vista policies.
Maintain accurate, timely, and complete documentation, including evaluations, daily notes, billing, progress notes, and discharge summaries.
Supervise Physical Therapy Assistants, students, and rehabilitation assistants in accordance with state practice regulations.
Provide education to patients, families, staff, and the community through in-services, home programs, and outreach activities.
Perform all duties in a safe, professional manner and complete additional responsibilities as assigned.
Provides functional supervision to Physical Therapy Assistants, students, and rehabilitation assistants.
Work Environment & Physical RequirementsThis position involves standing, walking, bending, lifting, pushing, and transferring patients. The Physical Therapist may be exposed to infectious diseases, emergency situations, and irregular hours, and may be required to wear protective equipment. Daily exposure to bloodborne pathogens may occur.
Minimum QualificationsEducation:
Graduation from an accredited school of Physical Therapy with successful completion of a supervised clinical internship.
Experience:
No prior experience required.
Licensure & Certification:
Current Illinois Physical Therapist license or license pending approval.
Basic Life Support (BLS/CPR) for Healthcare Providers required.
Additional Requirements:
Ability to provide care to patients of all ages.
On-call and call-in support required; travel to other Vista Health sites may be necessary.
