Opportunity Overview

SIH is looking for an Advanced Practice Provider to join our Primary Care team. If you're a provider that enjoys seeing all ages and working independently, this could be the opportunity for you.

Highlights:

1. 2 days per week (Th/Fr), 8a-5p, in Anna, IL
2. 3 days per week (M/T/W), 8a-5p, in Carbondale, IL
3. Experienced clinic staff
4. Epic EMR with Ambient Listening for efficient charting
5. Designated Rural Health Clinic

Qualifications:

• Current Illinois licensed Physician Assistant or Advanced Practice Nurse
• Current NCCPA (PA) or AACC (APN) certification
• Illinois Controlled Substance License or eligible to obtain
• IL DEA or eligible to obtain
• Current BLS certification
