Position: PM Clinical Supervisor

Position Summary

Eden Vista Burr Ridge, a a compassionate and resident-centered assisted living facility is seeking an experienced and highly skilled PM Clinical Supervisor to join our leadership team.

This role plays a key part in supporting the overall clinical operations during the 2:00 p.m. to 10:00 p.m. shift, ensuring that quality standards, resident care, and regulatory requirements are consistently met. The ideal candidate will possess strong leadership abilities, sound clinical judgment, and a passion for delivering exceptional care to residents.

Key Responsibilities:

• Provide oversight and supervision of nursing staff during the PM shift.
• Conduct assessments for new admissions, changes in resident condition, and discharges.
• Ensure quality resident care and compliance with facility policies, state, and federal regulations.
• Collaborate with interdisciplinary teams to support care planning and documentation.
• Assist in staff training, mentorship, and performance monitoring.
• Maintain accurate and timely clinical documentation using PointClickCare (PCC).
• Serve as a resource and leader to ensure a safe, efficient, and compassionate care environment.
• Participate in on-call responsibilities every other weekend.

Qualifications:

• Minimum of 5 years of clinical experience in a long-term care (LTC) or skilled nursing setting.
• Licensed Registered Nurse (RN) in the state of Illinois preferred.
• Strong clinical skills and knowledge of PointClickCare (PCC).
• Demonstrated ability to assess residents effectively, manage complex situations, and make sound decisions.
• Excellent communication, leadership, and organizational skills.
• Commitment to fostering teamwork and delivering quality outcomes.
