Job Description :
The Navigator will conduct outreach to justice and community organizations to receive client referrals for the Service Coordination program. The Navigator will be responsible for screening clients to determine enrollment eligibility. This position will connect clients to community resources and provide ongoing support to help young adults achieve goals and objectives.
Microsoft Office Skills

Assist young adult in creating goals and objectives Determine appropriate services to help young adults achieve goals Manage deliverables to meet expectations and deadlines Administer life skills Assessments Enter case notes and client information in a database Empowerment of clients in a manner that is supportive and objective Create transition plan for young adults as they complete goals Attend monthly meetings
