Overview
Physical Therapist – Mattoon, Illinois. Start Date: May 4, 2026. Duration: 13 weeks. Weekly pay range: $2,180–$2,332 per week. Guaranteed hours: 40 per week. Shift: 5x8 days (8-hour days). Work setting: acute care/inpatient rehabilitation environment.

Responsibilities
- Plan and execute rehabilitation programs to restore physical function, using therapeutic exercise, gait training, neuromuscular re-education, modalities, and manual techniques.
- Conduct patient assessments, develop individualized treatment plans, monitor progress, and adjust goals as needed.
- Document all patient care accurately in medical records and communicate findings with the care team; participate in rounds and care conferences.
- Educate patients and families on treatment plans, home exercise programs, safety precautions, and discharge planning.
- Maintain compliance with clinical policies, licensure requirements, and facility standards; ensure patient safety and quality of care.
- Demonstrate flexibility in scheduling (8-hour days with potential for 4x10-hour shifts) to meet patient needs.

Requirements
- Active Illinois Physical Therapist License
- Active American Heart Association BLS certification
- Minimum 1 year physical therapy experience
- Ability to work on-site in an acute care/inpatient rehabilitation setting
- Submission of 2 Supervisory Reference Forms completed with dates employed
- Profile materials: work history and skills checklist current within the past year
