Job Title: Patient Engagement Specialist - Emergency Room Liaison
Location: Southwestern Suburbs of Chicago, IL
Position Type: Full-time, Rotating Shifts, All shifts (Including Holidays and Weekends)

SECOND/THIRD/WEEKEND SHIFTS ENOURAGED TO APPLY
- Patient Identification and Engagement: Identify eligible accident patients in the ER, initiate contact post-stabilization, and provide an overview of the follow-up care services available within the hospital network.
- Cross-Department Collaboration: Collaborate with the hospitals care team to ensure a smooth handover of patient information for ongoing external care coordination, including medical and legal support.
- Educational Support: Provide patients with resources and information on the next steps in their care journey, including follow-up appointments, available specialists, and legal assistance if requested.
- Documentation and Compliance: Accurately document all interactions within our system, ensuring data compliance with hospital policies, privacy regulations, and patient confidentiality standards.
- Healthcare Knowledge: A solid understanding of healthcare practices in an emergency room setting and the importance of follow-up care coordination.
- Data Documentation: Experience in using electronic medical records (EMR) or other patient documentation systems within a healthcare setting.
- Interdepartmental Coordination: Ability to coordinate effectively with both ER staff and external case management professionals to ensure comprehensive patient follow-up.
- Empathy and Compassion: A strong commitment to providing supportive, patient-centered care in an ER environment.
- Communication Skills: Excellent verbal communication to explain follow-up options clearly, facilitate smooth patient handovers, and coordinate with case management teams.
- Detail-Oriented: Attention to detail in documentation and compliance, ensuring accuracy and consistency in patient data management.
- Adaptability: Flexibility to work in a fast-paced, high-stress ER setting, adjusting to rapidly changing patient needs and priorities.

- RN/LPN/LCSW Licensure or other Certification Required
