Physical Therapist - Travel Contract

Minimum Requirements
- Bachelor‘s, Master‘s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Illinois Physical Therapist license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Home Health experience preferred.
- Experience with OASIS documentation preferred.
- Experience with HCHB (Homecare HomeBase) electronic records preferred.
- Minimum education requirement: Bachelor’s Degree.
Assignment Details
- Facility Type: Home Health.
- Shift Name: Days.
- Schedule: 4x8 (four 8‑hour shifts per week).
- Weekly hour guarantee: 32 hours.
- On Call/Weekend Requirement: No.
- Coverage Areas: Clinton, Marion, Washington, Jefferson, and Hamilton counties; candidate should be prepared for travel between patient homes within these counties.
- Mileage reimbursement provided between visits from first patient to last patient.
- Required certification: Illinois Physical Therapy license.
- Must provide in-home physical therapy evaluations and related skilled services.
